These apply to all bookings including On-site Forklift Training, and Assessments.
FMS Training reserves the right to update and change the Terms of Service without any prior notice.
IMPORTANT NOTES
- “If you have booked your course, then you will have already accepted these Terms and Conditions”
- Please ensure you have booked into the correct course to prevent other avoidable fees. Please read the Terms and Conditions carefully.
Students must:
- Be at least 18 years of age at the time of assessment.
- Provide a Unique Student Identifier (USI)
- Provide three (3) forms of IDs. Please refer to the ID Checklist.
- Wear appropriate closed toe footwear (such as joggers, trainers, runners, or sneakers). No thongs/sandals allowed.
- Complete the enrolment form prior to the course start date.
- Be able to read and write in English (WH&S Requirement).
- Have an intermediate level of language, literacy and numeracy (to be able to complete the assessments).
- Be able to complete a written test (without assistance of a translator).
- Be able to complete a practical driving test.
- Have read and understood our Cancellation, Reschedule Policy, and Terms and Conditions.
- Have read and understood the pre-course materials (emailed at upon confirmation of enrolment).
Cancellation Policy
FMS Training is committed to providing students with a cancellation and reschedule policy that is fair and equitable. Consequently, the following areas are addressed:
All cancellations must be made in writing and sent via email to info@fmstraining.com.au accompanied by a supporting documentation. No course cancellation requests are handled over the phone.
Cancellations >11 Day’s Notice
A refund will occur if the candidate or group cancels their course at least eleven (11) working days prior to the scheduled course date, or in the case of an individual assessment. This will attract an administration fee of $150.00 (incl. GST) per person.
Cancellations <10 Day’s Notice
All cancellations made within ten (10) working days prior to the scheduled commencement date, or in the case of an individual assessment, will not be entitled for a refund. This is due to Workplace Health and Safety requirements regarding the scheduling of High Risk Licence assessments. (Please see ‘Substitute Delegates’ policy, as another option to cancel). Cancellation requests will only be approved at the discretion of FMS.
Cancellations during Course
Refunds will not apply to candidate/s or groups whose course has commenced or finished. Purchase orders/invoices must still be paid in full in the event that candidate/s or group did not show up and decided to cancel the course.
Cancellations due to Illness
In the case of a participant who cancels their course due to illness, FMS Training may, at its discretion, allow a refund of the fees. The following conditions apply:
- The participant must produce satisfactory evidence of the circumstances of his/her withdrawal, such as a medical certificate or statutory declaration (JP certified).
- An administration fee of $150.00 (incl. GST) per person will apply.
- Cancellation must take place prior to the commencement of the course.
Cancellations due to Emergency/Illness of a Trainer
In the case that an emergency or a trainer/assessor is ill, candidates will be rescheduled to another date which suits, or request to cancel their booking and request money back.
Cancellations Oral Assessment
Should a candidate mistakenly booked and paid the Oral Assessment online, when it is not actually needed; a refund will be provided less the Transaction Fee of $30.00 (incl. GST). Otherwise, cancellations and reschedules for Oral Assessments please see the Cancellation and Reschedule policies.
Reschedule Policy
FMS Training is committed to providing students with a cancellation and reschedule policy that is fair and equitable. Consequently, the following areas are addressed:

Reschedules >11 Day’s Notice
One (1) reschedule will be accepted without charge where FMS Training has been notified in writing at least eleven (11) working days prior to the scheduled commencement date. All subsequent reschedules at least eleven (11) working days prior to the commencement date will attract an administration charge of $150.00 (incl. GST) per person. All reschedules will be valid for six (6) months from the initial course date. Reschedule requests will only be approved at the discretion of FMS, and subject to course availability.
Reschedules 10 – 7 Day’s Notice
All reschedules made within ten to seven (10 – 7) working days prior to the scheduled commencement date, or in the case of an individual assessment, we will require the candidate to present a medical certificate or evidence of extreme personal hardship. Otherwise, an administration fee of $150.00 (incl. GST) per person will apply to reschedule the course. This is due to Workplace Health and Safety requirements regarding the scheduling of High Risk Licence assessments. Reschedule requests will only be approved at the discretion of FMS, and subject to course availability.
Reschedules <6 Day’s Notice
Once within six (6) working days of the scheduled course commencement date, or in the case of an individual assessment, should a candidate or group wish to reschedule a full training course or individual assessment item, this will be treated as a Non-attendance and a fee equal to the full course or assessment fee will apply. This is due to Workplace Health and Safety requirements regarding the scheduling of High Risk Licence assessments, and we cannot resell this place in the course. Please see the ‘Non-attendance’ policy.
Reschedules/Cancellations on the Day
Please see ‘Non-attendance’ policy.
There are no refunds offered on any cancellation of any rescheduled course, at any period of time.
Terms and Conditions
Deposit/Administration Fee
A non-refundable deposit of $150.00 (incl. GST) per person is required to secure their place in the course. This amount is included in the total course fee. Should a candidate request for a cancellation (see ‘Cancellation Policy’) and if a refund is approved, this non-refundable deposit can be used as credit (see ‘Credit’ policy) if candidate decides to rebook the course for another date.
Latecomer/s
Arriving late to a course affects the learning outcomes of all candidates. FMS Training reserves the right to refuse entry to candidates who arrive late to a course.
All candidates are required to turn up on time both on their scheduled training, and assessment. Latecomer/s on the assessment day will not be allowed to sit the exam, and will be treated as non-attendance. Please see ‘Non-attendance’ and ‘Resitting Assessment’ policies.
Insufficient IDs or Incorrect Footwear
Forklift candidates are required to provide three (3) forms of identification, one must be photographic, and wear appropriate footwear.
If a candidate has insufficient, no IDs, or is wearing inappropriate footwear on the day, this will be treated as non-attendance. Candidates with insufficient IDs, or incorrect footwear will not be entitled to a refund. Purchase orders/invoices must still be paid in full. To reschedule/rebook, the relevant fees will apply. Please see the ‘Non-attendance’ policy.
Non-attendance
If a candidate or group does not attend the scheduled day of the course they will not be entitled to a refund. Purchase orders/invoices must still be paid in full in the event that a candidate or group did not show up, and decided to cancel the course.
If a candidate or group would like to reschedule, they will be asked to present a medical certificate or evidence of extreme personal hardship (medical emergencies, death of a relative, natural disaster). Otherwise, a fee of $250.00 (incl. GST) per person for their rescheduled assessment will be incurred, a fee of $250.00 (incl. GST) to reschedule any Oral Assessment, and full course fee will apply to reschedule any training, as we are unable to resell that place in the course. Rescheduling based on non-attendance is at the discretion of FMS, and subject to course availability. Please see ‘Reschedule Policy’ section.
At the discretion of our Training Manager, a one-off opportunity to reschedule the course to another date will be provided, if the candidate can present a medical certificate or any other proof of extreme personal hardship. The Administration Fee of $150.00 (incl. GST) will apply per person.
IMPORTANT: A supporting document such as a medical certificate, or any other proof of extreme personal hardship is mandatory. This requirement is non-negotiable. No supporting document, no course reschedule. Therefore, if you are unable to provide one, the relevant fees will apply to reschedule.
Substitute Delegates
Substitute delegates are accepted without charge up to 6 months from the initial course start date. Please see ‘Credit’ section.
24-hour Cooling-off Period
A 24-hour cooling-off period will apply to all bookings. All cancellations must be made in writing and sent via email to info@fmstraining.com.au. No formal cancellations are handled over the phone. A refund will occur for cancellations made during this period less the transaction fee of $30.00 (incl. GST).
Early completion of course
If the candidate completes their course before the nominated timeframe they will not be entitled to any discounts or refunds.
Oral Assessment
Oral Assessment is available for candidates with Language Literacy & Numeracy issues; this will cost $250.00 (incl. GST) per person. candidates are required to complete the LLN Pre-Training Test. Oral assessment date will be subject to availability, and will be scheduled to a date and time available, and at the discretion of FMS.
Should a candidate be found Not Yet Competent (NYC) in any part of the oral assessment (either Knowledge, Calculations or Performance), a payment of $250.00 (incl. GST) Oral Assessment fee will apply to schedule another Oral Assessment.
Resitting Assessment
As of 11th May 2020, if a candidate is found to be Not Yet Competent (NYC) in any part of the assessment (either Knowledge & Calculations or Performance) no additional charge will apply for the first reassessment. One free resit must be scheduled within 60 days of the initial assessment. All resits must be scheduled via email info@fmstraining.com.au and initiated by the candidate. FMS will also do its best to accommodate the candidate for the next available time slot, but this will be subject to course availability.
However, should a candidate not pass their assessment on the second time, charges will apply for a reassessment at the reassessment rate of $250.00 (incl. GST). Assessment resits due to failure will be subject to availability, and rescheduled to a time available, and at the discretion of FMS.
Candidates undertaking a reassessment within 2 calendar months of the initial assessment are only required to complete those parts of the assessment for which they were unable to demonstrate competency (Knowledge, Calculations and/or Performance).
Candidates undertaking reassessment 2 calendar months after the initial assessment date must undertake the entire assessment (Knowledge, Calculations and Performance).
All reassessments must be undertaken within 6 months. Reassessment requests after 6 calendar months of the initial course date will no longer be approved. Candidates are required to rebook (full course fees apply) the Forklift Course to ensure candidates are brought up to the most recent theory and practical forklift driving practices implemented by WHS QLD.
On-site Forklift Course Resitting Assessment
If a candidate is found to be Not Yet Competent (NYC) in any part of the assessment (either Knowledge, Calculations, or Performance), resits will be conducted at 1/42 Paisley Drive, Lawnton QLD 4501. Please see ‘Resitting Assessment’ policy for further details.
Additional Training/Support
If the trainer believes that the candidate may need to study more or log more hours to become a competent operator, then the candidate will be welcomed back for more training prior to assessment. A $250.00 (incl. GST) fee for extra full day of training will apply.
Expired Assessment Summary Form
Individuals who have failed to lodge their Candidate Assessment Summary (CAS) within the nominated timeframe (60 days) will be required to retake all of their assessment items under Workplace Health and Safety requirements. No additional training will be provided and a reassessment fee of $250.00 (incl. GST) will be charged. Should individuals request training in addition to the reassessment due to the elapsed time since the previous assessment, then full course fees will apply.
Reassessment will be provided if booked within 6 months from the initial course date. If reassessment is booked after 6 months, full course fees will apply.
Promotional Codes
When FMS Training offers promotional codes, please don’t forget to type in the code and click on ‘check code’ when booking online. This will ensure that the special price will reflect upon checking out. FMS Training will not give any refund if an individual booked in and forgot to activate the promo code.
Credit
Any credit used towards an account will need to be used within 6 months, or else it is considered invalid.

I consent FMS Training using the information I provide and they subsequently gather, for the purpose it was collected, to assist in the administration of products and services and to carry out all necessary activities associated with their operational business activities, workplace compliance and legal governance issues. I understand that my student details may be viewed as part of an audit process by government officials.
