STUDENTS MUST:
FMS Training Services acknowledges the importance you attach to information that identifies you (personal identifiers such as name, address, date of birth, email address). We are committed to protecting and managing the personal information you choose to share with our organisation.
Through providing this information, we seek to ensure that you will be able to deal with our organisation in full confidence that your personal information will only be used by us in the ways we have described to you, that it will be held securely, and when there is no longer any legitimate purpose in retaining such information it will be disposed of appropriate.
I consent FMS Training Services using the information I provide and they subsequently gather, for the purpose it was collected, to assist in the administration of products and services and to carry out all necessary activities associated with their operational business activities, workplace compliance and legal governance issues .I understand that my student details may be viewed as part of an audit process by government officials.
FMS Training Services are committed to providing students with a refund policy that is fair and equitable. Consequently, the following areas are addressed:
*Payment in advance is essential to confirm a course booking. All course fees must be paid in full prior to the commencement of the course unless otherwise agreed to by FMS Training Services.
*A $150 administration fee will be charged for first time cancellations made 14 days in advance to commencement of the course if paid by cash, eftpos or credit card over the phone. A fee of $30 will be charged if paid via Paypal to cover the transfer fee.
*An administration fee of $200 per person will be charged for first time cancellations received less than 14 days prior to commencement of the course if paid by cash, eftpos or credit card over the phone, (to cover the cost of the administration and inconvenience to the business should this seat not be filled in the short space of time before the course commencing) plus an additional $30 fee will be charged if payment was made via Paypal to cover the Paypal transfer fee.
*No refund will be given for all cancellations received within 3 days prior to the course, on the day of the course, after the course has commenced, nor for withdrawals during the course. (Once you have made a booking you are occupying a seat in the course, and that seat cannot be 're-filled' if the student decides to cancel at this point in time. This costs our business, and this is why no refund will be given.)
*Students may elect to attend another scheduled course (a maximum of 2 rescheduled bookings are allowed) as an alternative to seeking a refund, provided vacancies exist on the nominated scheduled course, and the scheduled course is completed within 6 months of the initial payment.
*Students who have elected to reschedule their course will be given 2 chances to do so. If the student wishes to reschedule for a third time, that student will need to provide a medical certificate, otherwise no refund will be given and the full course fees will need to be paid.
*Students who have rescheduled their course and wish to cancel up to 3 days prior to the course will be charged a $250 administration fee, per booking, plus an additional $30 if the original booking was made via Paypal (to cover the Paypal transfer fee).
*Some participants may be able to complete this course before the nominal time frame and some participants may take a little longer, there are no additional fees or discounts on the course fees should this occur.
*Students will have 60 days, from the commencement of the course, to lodge their AS1 Form and Statement of Attainment to an Australia Post outlet for processing. Should the student fail to do this within 60 days, their AS1 and Statement of Attainment are no longer valid. An additional fee of $175 will apply and additional testing may occur.
*Students who wish to obtain a second copy of their Statement of Attainment, or students who have incorrectly completed their name on their enrolment form (needs to be as per drivers licence) will be charged an additional $10 to cover additional printing costs.
